| 6. | Automatic or Credit Card Payment Setup ACS offers a convenient way for you to mail in a credit card payment or sign up for ACS auto-pay™. |

| If you have the High-Speed Internet Bundle you will see: |
| 8. | Summary of ACS Bundled Services Detail of services included in the High-Speed Internet Bundle and the goVocal™ Bundle if applicable. These will be full month charges only. |
| 9. | Additional Monthly Services Any other services not included in the High-Speed Internet Bundle including prorated charges (partial month charges and credits) if applicable due to any changes you may have made to your account during the billing cycle. |
